Patient handoffs have been implicated in approximately 80% of serious medical errors. With work hour restrictions created by the Accreditation Council for Graduate Medical Education (ACGME), the frequency of handoffs has increased. The ACGME also requires residency programs to create protocols for handoffs. The Joint Commission developed guidelines for protocols to decrease in readmission rates and reduce on patient transfer times. We developed and implemented a standardized format for emergency department (ED) handoffs. We hypothesized that the new format would decrease handoff time while maintaining the quality of information transfer.
