On 25 March 2015, at 1330 hrs. Two crane mechanics went to PLWG for replacing boom sheave on gantry sheave walkway. After finishing the task on the gantry sheave walkway , the IP came down to the engine floor, his co-worker transferred tool bag down to the engine by using the rope. Once the bag reached the floor, the IP attempted to pulled the rope down a little bit in order to ease untie the rope from the bag. The other end of rope entangled with the handle of grease gun which was placed on the walkway, as a result; the grease gun fell off the gantry sheave walkway and ricocheted on 3 difference locations at crane ladder cage. While the IP looking up as warning was shouted from his co-worker, the grease gun continue fell and hit on IP’s nose , causing a swelling with a curve cut wound and mild bleeding on IP’s nose. After the incident occurred, the IP went down to cellar deck with his co-worker , washed his face and sit near the handrail for taking a rest. While he was sitting , there was a loss of consciousness (2-4 second). As a result; he lost his balance and fell backward. His head hit the floor, causing a minor contusion of scalp. His co-worker immediately woke him back and gave the first aid to him.
