Woolworths QA team in Thailand conducted factory focus on 14 January, 2015. Concerns on 3 main programs were addressed. Employees had lack of GMP/ Food safety awareness which might result in ineffective QA QC program and lead to the following observations.
1. Pest Control Program
- Lizards, Lizard faeces, birds, spiders in production area and packaging storage room.
- Yellow plastic curtain at the main entrance to production line was too short which can allow pest ingression easily.
- No defined criteria to evaluate the pest control effectiveness. Only raw data of pest inspection record was available but there was no data analysis in order to help evaluating the pest activity and implement the proper pest control program.
2. Cleaning Program
- Insufficient cleaning and monitoring programs, spider with spider web on wall of 1kg packing room, packaging storage, remained insects inside of lamps and lizard trap at entrance area.
- Cleaning schedule and inspection procedure are in place, the inspection result was recorded as clean and tidy. Daily cleaning is scheduled however in the actual production and storage areas were not clean. Accumulated dust and dirt were found on the packaging film (rolls)/ bags.
3. Foreign Object Control Program
- Found loose part (a bolt) on floor in rice bulk warehouse and there were a lot of temporary repair parts such as stick clear tape, plastic rope attached in filling room and machines.
- Found cracked glass cover at the destining machine. There was no incident record.
- The conveyer tunnel after the last critical control point (Colour sorting machine step) before going to packing machine, found some gaps (on the rice conveying pipe – Acrylic covers) which were big enough for a small lizard/ insects/ foreign object to get inside)
- HACCP Plan document system was not updated and critical limit only stated ‘to be in good condition’, there was no defined control measure while actual production and validation record showed different Critical Limit (CL), (seen destone step; actual production CL: 45 degree, validation record: 90 degree and operation parameter: 40-43 degree.)
- Foreign object detected machines – Validation of destoner machine was done without controlling parameter/ condition of destone machine and magnetic bar. Documented validation procedure was not available.
From the above situation, it reflects that the Corrective and Preventive actions were not properly taken in the last 6 months since we received the complaints. This will lead to potential for foreign object and pest contamination into our products which we don't want to have further quality incident anymore.
Within 19 Jan, please come up with the comprehensive, practical improvement plan with person in charge and timeline. Woolworths need to review your plan, once receive, we will have to follow up on-site again.
We have to consider the on hand PO your improvement plan is properly implemented and effective.
