- There is no formally approval for the first try and the incident happened (how to solve and make this as standard process as for the second try, IBM just asked for approval as per email below)
- there is no proper resource planning, no dedicate name of infra person who my team can directly escalate to in case the incident happen again. The incident happened on the other night is because my team could not get immediate response from the team who put the first debug program in and took too much time running around and at the end business got impact of improper preparation from Infra team
