To All Staff,
I am writing to you all to stress the importance of the safety initiative we have recently embarked on.
As you know K Linda joined us six weeks ago and her and the safety team have been working hard to close some of the gaps in the system of safety within our organization.
There is still much to be done but a few examples of the work that has been achieved this month are a full hazard register for the safety case which is a requirement under the SMS. In addition we now have an appropriate way in which to manage our changes to the organization with the new MoC process, and I request that you all follow this process rigorously.
As many of you know we have a new reporting system called QPulse and once again I request you all to report incidents and occurrences through this system. I request that all of you to take the time to become fully familiar with the system so that you can make a report with ease, or if you are a Department Head or hold a supervisory position that you delegate tasks through the system when you receive automated advice through the system. This could be anything from a TCAS warning to a hazard on one of the rigs or an inflight emergency – please ensure you identify and report issues that could ultimately save lives and assets. Remember if you don’t report something that you knew happened and something goes seriously awry then the just culture model would actually see that as a violation and you may be held accountable. If you see it REPORT IT.
You will see that there are a number RED boxes located in the hangar and canteen – these are to be used for the YOU SEE YOU ACT program – you may ask why we have moved to a paper based system and the reason is that currently we have no way to filter information within QPULSE so until we can rectify this we ask you to complete a paper form and submit via the locked RED boxes. The boxes will be cleared once per week and the issues will be handled by the S&Q team. You may also submit suggestions and recommendations for improvement into these boxes – this may include issues surrounding housekeeping , or an activity or procedure that you think could be done more expeditiously and efficiently. Please ensure you know the difference between a SAFETY report and a FLIGHT / TECHNICAL report as well so that they do not become confused within the system.
On another matter I want you all to understand that UOA will not tolerate any examples of what could be seen as bullying or discrimination in the workplace. We do not want to encourage a culture of fear whereby our staff feel threatened by supervisors or peers. If you experience a problem like this please ensure you submit a CONFIDENTIAL report through the QPulse reporting system or speak to the DMD or the Safety Adviser who will ensure your privacy is maintained in the first instance.
Working in a culture of fear (usually threats of loss of job) works against the safety of safety in that if mistakes or decisions that are made are treated in a bullying manner can often be seen as a systemic failure in a major incident or accident downstream. Please remember you all are part of the safety system – if you face any of these issues SPEAK UP.
Finally, if you notice within your own area of work that there is a lack of documented procedures please speak up – it is a requirement under the SMS to assure ourselves that the system is working and we do that by auditing. Therefore if an area has no documented procedures of how they do their job then it is impossible to audit and must be fixed up as soon as possible in the short term.
Please feel free to discuss safety issues not only with your supervisors but with the safety team – that is what they are here for – knowledge in this case should not be considered as power.
We must work as a collective team, without being protectionist about our own positions in our jobs with the main aim to provide our customers with a first class service, but SAFELY.
K. Padej and myself as well as K Nat are all 100% behind the safety initiative – it is imperative to our continued operation so if you have a question, problem or issue to raise please speak to safety team members and make sure you report through the various reporting channels that exist in the company and through QPulse.
Finally, I would like you all to put your thinking caps on and come up with an appropriate slogan for the company – this should not be longer than one short pithy sentence. Entries will be judged and the best one chosen and a prize for the best one will be awarded. You have until July 30th to submit a suggestion and I would ask that you send them to the S&Q email address so we can track and evaluate them.
