2.3. Commutabilityofreferencematerials
Commutabilityisdefinedas“theequivalenceofthemath-
ematical relationshipbetweentheresultsofdifferent
measurementproceduresforRMsandforrepresentativesam-
ples fromhealthyanddiseasedindividuals” [7]. IncaseanRM
is not-commutableanddemonstratesbehaviourthatdiffers
fromthatofnativepatientmaterial,variabletestresultswill
be obtainedforpatientsampleswithidenticalanalyteconcen-
trationswhenusingtheRMasacalibrator.Thismeansthata
non-commutablecalibrationmaterialbreaksthetraceability
chain.
Sample processingofRMcanresultinmodificationsofthe
measurandandmightberesponsiblefornon-commutability
[7]. Forexample,preparationofRMforlipidsandapolipopro-
teins accordingtoastrictprotocol(CLSIC37-A)hasshown
muchbettercommutabilitythancommerciallyavailable
patient serumpoolsorRMpreparedbyregularprocedures
[7,43]. Forseveralanalytes,whicharecurrentlytestedby
immunoassays,standardizationeffortsprovedunfeasible,
because ofthelackofcommutableRM(e.g., thetestsofcardiac
troponinI,B-typenatriureticpeptideandprostate-specific
antigen) [44]. Insuchacomplicatedsystemof(two-step)
immunoassays,molecularheterogeneityandminorchanges
in themolecularstructureorconformationoftheRMcan
influence thetestresultandcausenon-commutability.
ForstandardizationofqCCPmethods,commutable,value-
assigned secondaryRMs,traceabletoprimaryRMs,should
be developedinordertoproducetestresultstraceableto
standardsofhigherorder.Asmentionedabove,theuseof peptide
referencestandardsnecessitates,perdefinition,complete
proteolysistoguaranteefullmetrologicaltraceabilityofthe
proteinmeasurand.Inaddition,SIS-peptidesarecommonly
added after the digestionprocessanddonotcorrectforpeptide
instabilityduringproteolysis.Therefore,proteinquantitation
based on peptide referencestandardsshouldincludetheaddi-
tion ofSIS-peptides prior to digestiontoreducevariationsin
peptideresponse.Alternatively,theuseofmoresophisticated
SIS-strategies,suchaswingedpeptides [45,46], quantification
concatemers [47], concatenatedwingedpeptides [48], orfull-
length proteins [49,50] can beconsidered.
An additionalproblemwiththeuseofsyntheticreference
peptidesorproteinsistheabsenceofanalyte-freematrix
for preparationofcalibrationcurves.Inrespecttoallabove-
mentioned concernsaboutthecommutabilityofsynthetic
peptideorproteinreferencestandards,matrix-based,native,
and value-assigned,proteincalibratorswillprovidemostreli-
ableabsolutequantitationofendogenousproteins [51,52].
Because theproteinstandardsaresubjectedtoawork-up
that isidenticaltothatoftheunknownsamples,theyare
likelytocorrectforanyvariabilityinthe(pre-)analyticalwork-
flow.Theuseofa(single-point)matrix-basedhumanplasma
externalcalibratorhas,forexample,showntobeeffectivefor
the quantitationofapolipoproteins [51]. Obviously,absenceof
matrix-effects hastobeassuredbeforenativeproteincalibra-
tors canbeusedas(commutable)referencematerials.
3. Pre-analyticalqualityrequirementsfor
qCCP
In thecurrentlaboratorydiagnostics,70%ofdiagnosticmis-
takes aretheresultofpre-analyticalerrors [53]. Tothisend,
standardizationofspecimencollectionandsampleprepa-
rationinqCCPworkflows(Fig.1, leftandmiddlepart)is
pivotal.ThequalityguidelinesasposedbytheInternational
ConferenceonHarmonisationofTechnicalRequirementsfor
RegistrationofPharmaceuticalsforHumanUse(ICH)can
function asaframeworkfordraftingqualityrequirementsfor
qCCPworkflows(www.ich.org). Suchrequirementswillbedis-
cussed throughoutinSections 3 and4. Notethatthisoverview
leavesroomfordiscussionwhetherthequalityrequirements
for pre-analysisandanalyticaltestperformanceeitherhave
