Signs and Symptoms
Observe patients for pneumothorax symptoms 24 hours or longer after procedures associated with risk for pneumothorax.
Some patients (5%) with small PSP may not experience any symptoms for several days, or may experience mild symptoms (Ferri 2012; Havelock et al 2010). Patients with SSP will have more symptoms than those with PSP, even with smaller volume pneumothorax. Severe symptoms, including respiratory distress point to tension pneumothorax (Havelock et al 2010). Most patients with a pneumothorax will develop one or more of the following symptoms:
Apprehension, anxiety
Tachycardia
Shortness of breath, diminished lung sounds on the affected side, tachypnea, decreased SpO2
Sharp one-sided chest pain
Pallor, cyanosis
Subcutaneous emphysema (25%) (Thompson 2002)
Patients with a tension pneumothorax will experience:
Hypotension
Jugular venous distension
Significantly decreased lung sounds on the affected side
Sweating
Tracheal shift away from the affected side
Patients with mechanical ventilation may have a sudden decline of clinical condition with:
Increased peak and plateau pressures
Decreased tidal volume (Mason et al 2010)
