To uninstall the app from your device, tap and hold its icon on the Home screen until the icons wiggle, tap the "X" next to the app's icon to delete it, then press the Home button.
To delete the app from iTunes, open iTunes and click Apps on the left, press the Delete key on your keyboard, click the Delete App button in the dialog that appears, then click the Move to Trash button.
