A second conclusion is that the reform had a large
impact on the position of the stakeholders, their mutual
relations and culture (as summarized in Scheme 2).
In the supplier-market that held till 1994, where the
health professionals were the dominant group and pub-
lic health aims (like fewer injuries, less sickness and
better health) were the central items. It was character-
ized by an idealistic culture and a sense of cooperation
between stakeholders. Standard rates were charged and
there was mainly collegial control based on standards
promulgated by the professional association
