Operations must be issued by a Starwood Hotels
& Resorts Purchase Order
The monthly Work Order Contract Summary is to be
completed for each month. See Figure 1.
The Monthly Work Order Summary Form is
completed as follows:
a. Date of Issue - The date contract is issued.
b Contractor - Name of contractor/company.
c. Cost - Includes all labor, materials, taxes and
delivery costs associated with the contract.
d. Reason - A brief description of the work, i.e.
replace broken windows, roof repairs, condenser
water pump rebuild, etc.
