the last time (only time) I had this issue was when I installed a clean version of windows on my machine, but saved my Steam folder. What happened was the Windows Live Marketplace files became corrupt and caused the game to no longer function. The fix was to uninstall Windows Live Marketplace/GFWL completely, wipe it off the computer 100% using Add/Remove Programs from the Control Panel, and then reinstall it from scratch.
All you need is the Marketplace to be installed, once you've launched it once after the reinstall and seen it functions correctly, you should be able to just close it and launch Dark Souls as normal. GFWL may ask for your CD Key from within the game, if it does, you can retrieve it from Steam by right-clicking on Dark Souls from your Library, and choosing "view cd key", this option only appears if the game is currently installed. It may or may not ask, I think I had to input it, however the Marketplace shouldn't need to ask, as while the game "uses it" to some extent, you never need to run Marketplace on it's own besides to check if it's working.
