The Risk Prediction should be carried out by the Performing Authority on the day of work prior to requesting a PTW. This should be done with involvement of the work group and, where appropriate, the Area Authority. The purpose is to identify the:
• Hazards that will be present;
• Pre-permit measures to eliminate, or if not possible, minimize risks;
• Precautions required during the job to minimize residual risks;
• Post-permit measures to return the area into a safe condition.
The key findings of the Risk Prediction should be documented. The amount of detail required depends on the complexity of the job and the associated risks. Sites should provide tools or templates and training, to enable personnel to carry out this step. An example template is provided as a separate file with this GI.
In the case of larger and more complex jobs (and those carried out by contractors), a risk assessment and method statement should already have been developed, well in advance of the job, by those managing the work. This does not remove the need for a Risk Prediction, which allows the Performing Authority and the work team to review the job in the work area, prior to starting work, to make sure that:
• The risks have not changed since the original risk assessment was carried out;
• All team members are all clear on how they will do the job;
• All the equipment required is available.
A certain level of S&H competency is necessary to carry out a Risk Prediction. If the Performing Authority has not yet developed that competency, the Permit Issuer will need to lead this activity; however, this should still be carried out collaboratively, together with the Performing Authority.
