Best Qualified Candidates will have: • A bachelor's degree or higher in nursing from an approved school of nursing and six months or more of professional nursing experience • Licensure as a Registered Nurse issued by the state of Minnesota • Certification as a Public Health Nurse in the state of Minnesota • A valid driver's license with access to reliable transportation • Ability to visit clients at various locations within and outside of the metro area; flexible schedule to accommodate infrequent evening work assignments • Twelve months or more of community- based experience working with disabled individuals • Experience and knowledge of individuals with complex medical needs with proficiency in health and nursing assessments, and/or home care that includes private duty nursing OR experience and knowledge with individuals with serious and persistent mental illness and the services and networks that serve them
