1 Weaknesses Noted In Personnel Files Management
We had on November 5, 2013 reviewed job descriptions of key personnel and personnel files of
existing staffs based in Bangkok Office, Songkhla Plant and Sattahip plant for year
2013. Based on the review, we noted the following weaknesses:
a) Personnel files (“PF”) for all staff in Bangkok Office had yet to be maintained
We could not sight PF of Senior Accountant and QHSE Superintendent who are based in Bangkok Office.
b) Documentation pertaining to Personnel files maintained at Songkhla and Sattahip
Plants were not standardized.
Based on the company’s payroll as at October 30, 2013, the number of UOT’s staff was twenty-seven
(27) consisting of three (3) in Bangkok, ten (10) in Sattahip and fourteen (14) in Songkhla.
However, we noted inconsistency in the safekeeping of documents in the PF between Songkhla and
Sattahip Plants. It is important to standardize the documents to be retained in the PF
irrespective of the staff’s status i.e. Active or Resign.
Important documents which should be kept in the PF among others are:
i) Resume
ii) Application Form iii) Interview Form iv) Offer letter
v) Medical Report
vi) Copy of Relevant Certificated vii) Promotion / Transfer Letter viii) Yearly Appraisal
ix) Trainings attended
x) Exit interview Form – resign staff xi) Resignation Form – resign staff

Audit Comment/Recommendation (MH)
To establish clear guidelines on staff recruitment, confirmation & resignation process and
ensure that important documents are kept in the files for reference purposes.
Management Action Plan
Normally, UOT has the personnel file for all staff in Bangkok and will keep the copy at the
Sattahip or Songkhla based on the location that the employees has worked except for the Director
General that has been kept by the ex-Boss since he has worked in Labuan before transfer to Shah
Alam and KL. For BKK the QHSE Superintendent and Senior Accountant are the new staff that just
joined UOT around the mid of 2013 that may be not know where are the file. As when we interviewed
them we also have the interview form, application form, certificate, resume, appointment letter
that Director General gave to the Finance & Admin Executive to keep in the file with her
resignation form and she has not inform where to keep.
UOT will find and collect all personnel file to keep properly. Plan to finish by November
2013.
