2015-06-01 Improvement on flammable and combustible gas/liquid handling (Severity: High)
1. Relocate emergency manual shut off valve of liquefied petroleum gas (LPG) vessels (Severity: High)
During the survey, we found that the LPG vessels are equipped with an emergency manual shut off valve. However, the emergency manual shut off valve is installed very close to the LPG vessels.
In the event of a fire, this circumstance may lead to unnecessary time delay or unable to activate the emergency manual shut off valve.
Therefore, it is recommended that the emergency manual shut off valve should be positioned where it can be safely and immediately activated. In addition, it would help the staff to prevent the possibility of confusion or misuse when the emergency manual shut off valve is needed, the emergency manual shut off valve should be clearly marked. They should be marked conspicuously to indicate the location.
