Here is the steps for members to follow on how handle the account transfer application form,
1. Print the form
2. Fill up the blanks (hand write)
3. put a signature or both parties signature(we do not accept "write name" as signature.)
4. Attach both Identification Card. (It would be a good practice for the picture of Identification card is remarked as "For Verification only")
5. witness signature must be leader/upline.
6. Send back to us process with a legible and full page attachment, must sure complete all the relevant information.
Remark: Your request form are subject to management approval.
