Clients considered to be at moderate or high risk for malnutrition through screening are usually referred to a dietitian for a comprehensive nutritional assessment to identify specific or confirm the existence of malnutrition. Nutritional assessment is more accurately called the nutrition process, which includes four steps. While nurses use the same problems solving model to develop nursing or multidisciplinary care plans that may also integrate nutrition, the nutritional plan of care devised by dietitians is specific for nutrition problems.
