Dear all,
Although we did make good intentions to have a safe 2016, the first 10 days of the year turned out to be extremely negative on safety.
In the past days two serious accidents took place on KTN terminals; in both cases a pedestrian colleague was hit by a vehicle.
Awaiting further inquiry and reports, we already want to alert all of you
• to check your working environment,
• to make Safety Observations on behavior and work method,
• to reinforce and impose well-known measures
PEDESTRIAN OPERATOR HIT BY REVERSING FORKLIFT
On Saturday 8th Jan a packaging operator was hit by a forklift and injured his foot (ankle sprained). While the forklift (driven by customer’s driver ) lifted a hopper loaded with product (after having stopped a leak from this metal box hopper), he unexpectedly drove backwards (approx. 3-4 meters). Our operator was at that time busy with cleaning up the spill on the floor, and was hit by the reversing forklift; he fell on the ground and his right leg got squeezed under the forklift.
Clearly the interference on same working zone of forklift and operator + not respecting the priority for pedestrians + unsafe driving will be the main root causes of this accident.
