Thank you for the document, could you also please send us over the below pending requested document so that we can proceed with the name change process.
1. A completed Name Change Request form for the transferring of user accounts (Please see attached).
2. Name Change Notification Template:(Attached) The Template needs to be updated under the Company Letterhead, signed by either VP or Anyone from the Legal Department. Letter must be updated with the date and New company information wherever necessary. (Send it back in PDF format)
3. We have attached the license information that could not be accommodated in the ACF form in a separate document. Kindly provide us with a signed copy of this document
