* No monitoring of refrigerator temperatures
* Refrigerator not locked or kept in locked room
* No regular checks on emergency medication
* Out of date emergency medication
* Evidence of using out of date sterilizing tablets
* Prescription pads not logged and recorded
* Prescriptions loaded into printers that are accessible by all staff -- and patients
* Drugs prescribed for named patients being used as stock drugs in the surgery
