Direction: How to complete this Post-Assessment
1. Employee should select the 'SDP-Post Assessment tab' enter their name, program date, business and manger's name.
2. The employee should then proceed to complete the "participant" section by rating themselves on a scale of 1-5 on how closely they match the skills listed.
3. Once the participant has completed the self rating, the participant must forward the file to their manager.
4. Once the manager receives the file, the Manager should complete the "manager" section of the pre-assessment by rating the employee on a scale of 1-5 on how closely they match the skills listed.
5. After completing their rating of the employee, the manager is responsible for scheduling a one-on-one meeting with the employee to discuss the key difference in the ratings, what changes they have identified after the completion of the training, the employee's key learnings of the training, and the manager and employee expectations on how the employee will continue to develop the skills acquired during the training. Be sure to document key discussion points, so that it is easier to reference in the future.
6. After the manager and employee have completed the assessment and have had their one-on-one discussion, the manager should forward a copy of the completed pre-assessment to me (with the employee in copy) within 21 August 2015.
