Objective :
Demonstrate adequate hydration as evidenced by stable V/S and increase intake of fluid.
Expected measurable outcomes :
1. Vital signs are within normal limits. T=36.5-37.5 OC, PR=60-100 /min, RR=12-20 /min, BP=90/60-120/80 mmHg.
2. Peripheral pulse can be palpated.
3. Skin turgor and capillary refill good.
4. Balance of urine output.
5. Normal electrolyte levels.
- Serum sodium level is 136-145 mEq/L.
- Potassium level is 3.5-5.1 mEq/L.
- Chloride level is 101-109 mEq/L.
