Self-Assessment Tool
A. Introduction
Fire Safety is one of the basic requirements in adidas Group Workplace Standards. A fire incident in the factory might cause serious harm to properties, product and more critically the workers. For this reason, SEA continues to take measures which might help our suppliers to improve the fire safety condition in their factories, hence the introduction of this Fire Safety Risk Rating Self-Assessment Tool (‘Tool’)
The Tool emphasizes upon key elements and critical areas of fire safety, including the design of the building, fire- fighting equipment, fire prevention etc. It will help to determine overall risk level in the factory pertaining to fire safety system. The Tool has been developed with various questions and combination of variables and uses the Bangladesh Safety Accord standard as reference.
By its name, it should be self-explanatory that the supplier is required to do this risk assessment exercise using the Tool by themselves. The outcome of this exercise will help the factory, and SEA team, to understand which category factory will fall into out of four categories, i.e Very High Risk, High Risk, Medium Risk and Low Risk. The information will be kept confidential while we strongly recommend our supplier to provide correct and complete information.
Note: This self-assessment exercise does not intend to cover all aspects of general fire prevention including sources of ignition. Nor does it cover the prevention of accidental injuries.
Nothing in the tool shall be construed to prohibit better types of building construction, such as more exits, better equipment or otherwise safer conditions than the minimum requirements specified in the tool.
B. Objective
• To understand the risk level pertaining to fire safety in the factory
• To develop corrective action based on the outcome of the self-assessment exercise
• To upgrade the readiness and effectiveness of fire safety system in the factory
• Raise awareness among the factory management team about their factory’s fire safety status.
C. How – To: Guidance to do the Self-Assessment (using the Tool)
Factory senior management is to designate the Safety Manager or Officer to undertake this self-assessment exercise as the Assessor. In the absence of Safety Manager or Officer, the person in charge for maintenance may also be designated to carry out this exercise so long as he/she is familiar with technical aspects of fire, electricity and building structure of the factory.
There are 2 tabs (worksheets) in the Tool:
1. General Risk Factor:
• This is the first step which needs to be completed: fire hazard classification. Assessor only needs to click the appropriate box based on the type of manufacturing and number of workers.
• For the number of employees: it shall be the total number of workers employed in the factory, irrespective of the shifts system.
2. Risk Assessment Tool:
• Complete the general information in the provided column in the top section (factory name, assessment date, and assessor).
• There are 4 of section in this Tool: (i) Legal Compliance (ii) Fire Prevention measures (iii) Fire Protection System and (iv) Evacuation and Emergency Preparedness
• All questions from each section must be completed by choosing the appropriate options provided in the tool. Assessor does not have to complete all the questions in sequence; most of the questions will require proper analysis and site inspections for verification
• Most of the questions are equipped with ‘Comment box’ which will automatically pop up when the cell is clicked. The information in the Comment box will help assessor to understand better the questions and the requirement.
• In addition, there is specific explanation about the Status options provided in the tool so that the Assessor can choose the most actual, relevant and appropriate options. This explanation can be found by clicking the icon in the ‘Explanation’ column provided for each questions (far right column)
• Once Assessor has completed all the questions, then we will find the outcome at the bottom which is the Risk Score and the classification of the Risk Rating. The higher is the Score means the fire risk is also higher.
• For each questions, factory shall fill in the ‘Plan of Action’ column for any gap or non-compliances identified during the assessment
• Finally Assessor shall insert his/her name at the provided box in the bottom, and most importantly also inserting the electronic signature of the factory Senior Management for their acknowledgement to ensure senior management is fully aware of this risk assessment exercise and the outcome
NOTE: Please do not put anything or change anything on worksheet with name: “Sheet 1” in the Offline Fire Risk Assessment tool.
D. Timeline and Submission
• Factory shall complete this self-assessment exercise by mid of September 2015 at the latest.
• The completed tool shall be submitted to your respective sourcing contacts. Factory shall also submit supporting evidence such as the valid legal permit etc.
• Your Sourcing contact will then input the score, uploads the tool and the supporting evidence into FFC accordingly.
