Description of the organization and its target audience
I am an author and Registered Respiratory Therapist, Neonatal Pediatric Specialist. My book tells for the very first time the story of Patrick Bouvier Kennedy, the infant son of John and Jacqueline Kennedy, who died August 9, 1963 after thirty-six hours of life from a common lung ailment. A 12 chapter book, the first 6 chapters chronicles the history of infant care and the last six chapters are devoted to Patrick’s short life and how his death would usher in the modernization of newborn care.

Description
I want designed a book cover with the following title and subtitle followed by my name and credentials:
Patrick Bouvier Kennedy,
The Legacy of a Brief Life and a History of Newborn Care
Michael S. Ryan, RRT - NPS
I have the idea of including three images on the front cover, as follows:
A (1st image) or ideally a sketch image of the President and Mrs. Kennedy standing or (they were catholic) kneeling with backs turned facing away from the reader while looking at (image) empty crib. I would also like to include a stethoscope (3rd image) to be included as a medical symbol artfully juxtaposed onto the cover.
Of course, alternate suggestions along these lines are welcome.

Notes
If a designer should choose an image rather than sketch of the president and 1st lady, I’m obviously concerned about copyright laws, though will consider any photo of the two in the public domain. In any case, intuitively I think the sketch would really be nice in order to better capture the readers imagination though I need to be clear I'm not the expert in book cover art.
Being the book will be self-published I understand the publisher requires image resolutions of more than 300 dpi.
