'- Alcohol with concentrate 70% and 98%, included as flammable liquid, were contained in spray bottles at micro-biological laboratory without label and symbol to warn workers about its hazards. Chemicals that were not attached with label and symbol were found as well as spare-part warehouse of maintenance area.
- A bucket containing diesel fuel was left open at spare-part warehouse of maintenance area, may cause spills and related hazards e.g. reaction with the chemicals or slips and fall.
- Secondary containment of lubricant (Drytech 1, corrosive) used at Production line had hole at the base of the secondary containment which may release spills of chemicals outside the secondary containment.
- Flammable cabinet at preform injection room contained not only flammable liquid but also non-flammable liquid packed in carton box which was combustible materials. There were plastics sheets and plastic tray as well in the cabinet which were combustible materials as well.
- Spill kit for spill handling at diesel fuel dispensing station as well as fleet workshop i.e. sand and saw dust was found very limited (level already almost reached the base of the drum used to store sand/saw dust. This small amount of sand/saw dust was considered not sufficient to be used in emergency condition.
- Bunker oil tanks (combustible materials, 2 tanks) was inspected in July 2012 by external licenced party. It was seen from the report that some points of checking did not meet the minimum specification i.e.
For tank no. 1 - ST00-0-371:
Right side, point no. 5 - result of measurement: 4.6 mm, < standard 4.7 mm
Left side, point no. 8 - result of measurement: 4.6 mm, standard 4.7 mm
Back side, point no. 2 - result of measurement: 4.6, < standard 4.7 mm.
For tank no 2:
Right side, point no. 3 - result of measurement: 4.6 mm, < standard 4.7 mm
Left side, point no. 9 - result of measurement: 4.6 mm, standard 4.7 mm
however the result of measurement of these points were Pass so actions taken by the Company.
Note: the bunker oil tanks have been provided with secondary containment.
- A can of Technomelt Pur Cleaner found under the working table at Line 7. There was no MSDS available at the point of use during the audit. MSDS was only presented after requesting the document from supplier.
