To a large extent, care following cancer treatment is typically focused on surveillance for
disease recurrence. Despite the importance of this objective, other significant health problems
may persist or become apparent later in the course of the cancer trajectory. In this setting, the
success of the initial cancer treatment may be undermined by failing to anticipate and address
later detrimental health effects. Establishing a Survivorship Care Plan has been suggested as
one way to prevent the disconnect between successful initial cancer therapy and sub-optimal
long-term follow-up care
