We are the claims settling agent appointed by the cargo insurer, PT Asuransi AXA Indonesia, to manage your claim.
We were advised by our surveyor from McLarens that 1 piece of Service Part (Tray AGHN 050) was dented. Based on the invoice, the loss is calculated to be EUR81.60. In view of the loss amount, we have not arranged for any site survey, but instead would request that you submit the following documents, in order for us to handle your claim accordingly :
1) ORIGINAL Insurance Policy/Certificate
2) ORIGINAL Bill of Lading/Airwaybill
3) Commercial Invoice
4) Packing List
5) Claim Bill (statement quantifying your claim)
6) Photographs of the damage
7) Any Other pertinent documents (e.g. Port Survey Note, Equipment Interchange Receipt, Delivery Order)
In addition, kindly also complete as detailed as possible the attached Claim Form for submission to our office together with the above documents as soon as possible.
