Attached please find Probation Evaluation Form of your team member. Kindly review and evaluate your subordinate according to the step as follows :
Steps of Evaluation:
Step 1 : Provide this form to subordinate for self-evaluation first while Evaluator also assess his/her performance
Step 2 : Set Appraisal Meeting & Feedback in order to get probation evaluation feedback and final outcome for employment action
Step 3 : Sign on probation evaluation form
Step 4 : Submit to HR Department at K. Suthathip by 7 September 2015
REMARK/PRIORTY
In case that line manager delays to submit Probation Evaluation Form, there will be consequences to the employee’s benefits and certain legal related issues as follow;
1. Employee status
2. Benefits (Provident fund, Vacation Leave Activation , Dental & Accidental Claim)
3. Work Process which is related to labor law (ex. Termination process in case of fail probation)
4. Salary adjustment (if any, as agree upon hiring)
