Ethnopharmacological relevance:Maekmoondong-tang (MMDT) isatraditionalherbalmedicationwidely

Aim ofthestudy: This studyisaimedtosystematicallyreviewtherelevantrandomisedcontrolledtrials
(RCTs)todeterminetheeffectivenessandsafetyofMMDTforcough.
Materials andmethods: Electronic andhand-searchingof7databases(CochraneLibrary,MEDLINE,
EMBASE, OASIS,RISS,CNKIandCiNii)wassystematicallyconducteduptoFebruary2015forRCTstesting
MMDT inpatientswithcough.Theprimaryoutcomewascoughsymptomimprovementusingcough
diary,visualanalogscale,orresponserate.Riskofbiasoftheincludedtrialswasevaluatedwiththe
Cochrane riskofbiasassessmenttool.Thedichotomousdatawerepooledtoobtainariskratio(RR)of
cough persistingaftertreatment,with95%confidence intervals(CI).
Results: NineRCTsinvolving2453participantswereincluded.Themethodologicalqualitywaslargelypoorfora
majorityofthestudies.MMDTreducedtheseverityofcough by74%comparedwiththeconventionalantitussive
medicationsinvariousconditions(n¼1145;RRofcoughpersistingaftertreatment¼0.26;95%CI,0.19–0.34,
I2¼0%).TheadditionofMMDTtoconventionalmedication inpatientswithpostinfectiouscoughsignificantly
alleviatedsymptomsuptoday5buttheeffectwasnotmaintainedoneweekafterwards.Forotherdiseases/
conditions includingchronicobstructivepulmonarydisease,lungcancersurgery,andasthma,theevidenceis
inconsistentforMMDT/mMMDTforcough.Adverseeventsappeartoberarebutthereportingwaspoor.
Conclusions: The currentevidencefromoursystematicreviewandmeta-analysisonMMDTforcoughisin-
conclusiveandweproposethatrigorouslydesigned,placebo-controlledtrialsofMMDTshouldbeconductedto
establishitsplaceinmanagementofcough.
