The information was then compared and differences noted. Differences highlighted consisted of information that had either changed between notes or details or deemed from the reviewer’s professional opinion to be pertinent, in the pre-hospital notes that had not been transferred to the hospital notes.
Information not recorded in the ambulance service notes but subsequently reported by the Emergency Department was not included in the results. The subsequent gathering of information was not part of this audit. In addition it could not be determined how other factors such as time for assessment, treatments, investigations available or witness availability may have influenced the hospital assessment.
