It was found that there was some mis-communication between Sales rep and Sales secretary. I was told by warehouse manager, Aree, that when she got the returned products, the document from Sales team stated as “damage”, then she managed it under normal process for damage products.
“Damage” is commonly understand at local work level that product haven’t been used with patient due to some certain reason such as product may be dropped during the operation which resulted in sterility of the product or the wrong size/in-appropriate size may be opened. This claim mainly involved with finance matter as when damage products are treated as scrap, company has no need to pay local VAT (align to local VAT regulation).
That’s why it was not reported at the time when incident occurred.
But just recently that we got the same returned products for the same reason. For this time, Sales supervisor mentioned that her team need to filled-in PER but why not previous incident. This brought to Aree’s attention, she then consulted with me. So, we asked the sales rep to complete the PER and sent to you then.
