• Allow each member of the panel to have a template at their place that they can fill in as the process moves along. This is a way to provide panel members with quick visual references and more involved. This should not replace the facilitator's documentation. The facilitator needs to ensure consistency and provide the official results of the Safety Risk Assessment
• The facilitator must understand that personally capturing focused comments does not replace the need for a dedicated note taker to capture the detail of the discussion and the decisions made.
It is very difficult for the facilitator to both take notes and effectively facilitate during the Safety Risk Assessment . SRAs may benefit from having a recorder or note taker to free the facilitator to focus on keeping the process moving and productive.
At the conclusion of the Safety Risk Assessment all notes should be gathered and pulled into a draft report as expeditiously as possible. This ensures the capture of the information while it is still "fresh in everyone's minds. Refer to Appendix Dfor sample final report templates
