Nina’s husband, to whom she has been married for 10 years, brought her to the emergency department. He was attentive to her, spoke reassuringly, and appeared concerned about Nina’s condition. According to her husband, a day ago Nina slipped as she was getting out of the bathtub. When she slipped, she bumped her head on the faucet and then fell on her arm. As her husband spoke, Nina sat quietly with her head down. She cradled her right arm and appeared to be in severe pain. Her eight eye was red and swollen shut , and she seemed to have some difficulty breathing. Despite protests from her husband, the nurse inter viewed Nina separately from her husband in a private consultation room. Although the nurse inquired directly whether Nina's husband had beaten her, Nina denied the abuse. The physician performed a complete physical and neurologic examination and took series of x-ray films. When Nina returned to her room, the physician told her that she had fractures of the wrist, facial bones, and several ribs. The x-ray films also indicated multiple old, healed fractures of the ribs and pelvic girdle. The nurse spent time explaining to Nina how unlikely it was for her injuries to result from a fall in the bathtub. The nurse also reassured Nina that nothing she could have done would deserve such abuse by another person. Nina finally acknowledged that her husband had abused her nut insisted she had no intention of leaving him because he was a good husband. According to Nina, the only time her husband is abusive is when she fails to fulfill her domestic responsibilities and therefore provokes him into losing his temper and beating her. Nina has no children and no family nearby, except for a younger sister who is currently over whelmed with her own family problems. Nina is psychologically and economically dependent on her husband and has no means of financial or psychologic support.
