5. Certified copy of annual corporate income tax returns (Phor.Ngor.Dor.50) for the fiscal latest fiscal year ended together with its tax payment receipt, which has been certified by the Revenue Department.
6. Certified copies of the monthly withholding income tax returns (Phor.Ngor.Dor.1) of both Thai and Alien staffs for the months together with theirs tax payment receipts, which have been certified by the Revenue Department.
7. Certified copy of a sketch map showing the location of company offices in Thailand
8. Certified copy of two Authorize Directors of the company’s ID card

- A photo showing the office building.
- A photo showing the company’s name / office sign.
