History of current condition was obtained directly from patient interview and is as follows: On October 25,
2012 patient indicates he received a combined pneumonia and tetanus (DTaP) vaccination. 9 days later the
patient experienced right upper extremity weakness while attempting to lift a deer carcass into the freezer,
but dropped it due to weakness. The next day he remembered having “significant pain in all joints.” From
that point, he became very fatigued with his normal level of activity but was still able to carry on with his
normal work-related duties of feeding cattle, driving, and feeding himself as well. Mr. K experienced his
