Discharge Planning
• Refer the patient to a care navigator or case manager for care coordination.
• Refer the patient to a genetic counselor for BRCA-1 or BRCA-2 testing and counseling as appropriate.(Katapodi et al).
• Encourage participation in community support groups.
• For women with recurrent ovarian cancer, encourage participation in support groups with women in similar circumstances.
• Provide community resources for the patient as prescribed.
• Provide community resources for family members.
• Provide the patient and family with education about the therapeutic regimen as prescribed.
• Provide reliable printed and online educational and support resources for the patient and family, including:
o Cancer.Net
o American Cancer Society
o Gilda's Club (Clubs in a variety of cities use the URL gildasclub followed by the city name, eg, www.gildasclubdetroit.org)
o National Ovarian Cancer Alliance
o National Ovarian Cancer Coalition (NOCC)
Evaluation
• The patient will be free of complications of chemotherapy.
• The patient will be free of complications of surgery.
• The patient will have minimal or no pain.
• The patient will be free of nutritional and fluid imbalances.
• The patient will exhibit appropriate coping skills.
• The family will exhibit appropriate caring and coping skills.
