Assess and treat any underlying causes of pain. Consider paracetamol, ibuprofen. If not sufficient use morphine
by the route best tolerated by the baby: oral, buccal, intravenous or subcutaneously. (refer to CBNF and
symptom control manual ACT). Increase doses by 30-50% as needed to ensure the baby is comfortable. There
is no upper limit to the dose of morphine for relief of pain or distress even in the knowledge that this may cause
respiratory depression (double effect). Buccal or subcutaneous midazolam can be used if morphine is not fully
effective. However it is illegal to give any medication with the main intention of hastening death.
