Ineffective Breething Patterns
Objective : To protect hypoxia.
O : Patient have COPD.
O : Listen to the sound of wheezing lungs.
O : Rate 28-32 BPM
- O2 saturation 96%
- On O2 cannula 3 LPM
Plan and Outcome :
- Respiratory rate normal is 16-22 BPM
- O2 saturation 95-100%
- No hypoxia such as peripheral cyanosis.
- Absence of diminished breath sounds.
Nursing intervention :
1. Observe cyanosis such as peripheral cyanosis and restless.
2. Suction clear airway when a lot of mucus by use
3. Record vital sigh and oxygen saturation q 2 hrs.
4. On oxygen cannula 3 LPM
5. Absolute bed rest
