if dimerization is the suspected cause of biclonality, incubation with 2-mercaptoethanol can reduce the bonds leading to dimerization and can resolve these cases into a single peak.101 A comparison between human monoclonal gammopathies and biclonal gammopathies found no significant difference between clinical presentation, prognosis, or treatment response.106 The distinction of a biclonal pattern from a monoclonal pattern and the distinction of a truly biclonal gammopathy from a dimerized monoclonal gammopathy is interesting from an interpretation standpoint and could have an effect on monitoring protocols, but there is likely not a significant clinical difference between dimerized monoclonal proteins producing a biclonal pattern and a true biclonal or monoclonal gammopathy.5
