contracts do not exist, or, where necessary services are not covered, sites are responsible of identifying and
nominating potential Waste Management Contractors to their Business Unit EHS Director for their assessment
and approval. The nomination shall include the necessary documentation to support the requirements of
Section 5.3 and justification for use. The facility shall present in their nomination a written justification for
selecting the corresponding Waste Management Contractor, including compliance assurance, fiscal and waste
management benefits.
The Waste Management Contractors may be used only after evaluating its programs following Section 5.3 of
this standard. These checks should also be carried out periodically, with frequencies not to exceed three years,
to ensure authorization has not lapsed, and waste handling conditions remain adequate. Inspection and
auditing can be carried by third parties, such as CHWMEG and WFAA, which are providers authorized by
Corporate EHS.
Note: CHWMEG is an organization in the US that performs third party audits of Waste Management
Contractors, mostly in North America, and limited participation in EU, South America and APAC. WFAA
is a similar organization located in the UK that performs these audits in the EU.
5.3 Elements of Waste Management Contractor Evaluation
The person(s) conducting the review of a Waste Management Contractor shall ensure the elements listed in
this section are covered, and proper documentation is obtained from the audited facility, to substantiate a
decision of use or declination of use.
5.3.1 Determination of Ownership
Information regarding the legal owner and operator of the facility to be assessed shall be collected. This
includes:
• Name and address of legal owner and operator
• Ownership status of the facility and number of years at the facility
• Name and address of parent company
• If facility is leased, name and address of landlord
The Waste Management Contractor shall not be approved if there is not a clear evidence of who is the legal
owner or operator.
5.3.2 Site History
The history of the facility use and ownership shall be assessed to determine the possibility of historical
contaminations which may have an ongoing impact. This includes inquiring how long the facility has been
operated as a Waste Management Contractor, what were the prior uses of the facility and who the former
owners and operators were.

5.3.3 Fiscal Solvency and Insurance
It is necessary to collect recent information indicating the financial conditions of the Waste Management
Contractor to determine its fiscal solvency, as well insurance coverage. This includes the review of financial
statements, including reserves for environmental liabilities and asset retirement obligations, as applicable and
insurance documents including:
• Comprehensive general liability
• Workers compensation
• Automobile
• Pollution liability
• And any other regulatory required insurance coverage
The Waste Management Contractor shall not be approved if the review of this documentation shows that the
facility liabilities are higher than its reserves and assets or its financial history is weak.
Note: Financial conditions documentation shall be evaluated by Covidien Vice-president of Legal/Chief
EHS Counsel to determine their approval.
5.3.4 Authorized Waste Activities
The facility activities shall be evaluated to determine their compliance. This includes:
• A description of the facility operations
• List of regulatory agencies that have jurisdiction over the facility at national/federal, provincial/state
and local levels
• List of permits, licenses, registrations and authorizations under which the facility operates and their
current status
• List of authorized waste classifications which the facility is permitted to manage
• List of actual waste classifications that the facility is managing
• Waste acceptance procedures
• Waste receipt and management procedures
• Waste treatment and disposal methods
• Agencies inspection records and status of corrective actions where applicable
• Off-site waste management where applicable
• Subcontractor relationship, including the Waste Management Contractor’s duty of care to ensure its
subcontractors are compliant with applicable regulatory requirements
The Waste Management Contractor shall not be approved if it is not authorized to manage the waste generated
by the Covidien facility, or if any necessary permits, licenses, registrations or authorizations are missing or
expired without evidence demonstrating its renewal or reinstatement.
5.3.5 Compliance History
The facility compliance history shall be evaluated to determine its ability to meet applicable regulatory
requirements and its status before regulatory agencies or authorities. The review shall include:
• Involvement in any removal, remediation, or other environmental clean up action
• Involvement in any lawsuit relating to the work performed at the Waste Management Contractor
• Government agency enforcement action relating to environmental contamination of the real property
• Citations for violations of any national/federal, provincial/state, or local laws or regulations from:
o Environmental agency or authority
o Occupational safety and health agency or authority

The review shall cover no less than three (3) years of the facility’s compliance history.
The Waste Management Contractor shall not be approved if there is evidence of significant violations of
regulatory requirements, recurrent violations of any regulations, or repeated notices of violations or regulatory
agency actions.
5.3.6 Waste Management Contractor Client Base
Where allowed by confidentiality requirements, the facility review shall include an analysis of the Waste
Management Contractor client base to determine the type of industry served by the facility, and the types of
waste being received by the facility. The evaluation shall include:
• A list of the top ten (10) customers with contact information
• Percentage of revenues generated from Fortune 500 companies
• Percentage of revenue that would be generated from Covidien
The evaluation shall also review any customer complaints received in the three (3) years prior to the
evaluation date and the respective resolutions or corrective actions.
5.3.7 Environmental Releases, Spills and Emissions
The Waste Management Contractor evaluation shall include a review of environmental releases or spills that
have occurred in the five (5) years prior to the evaluation date and the respective resolution or corrective
actions. Documentation of any remediation activity shall be reviewed to determine compliance with
applicable regulations or orders from regulatory agencies or authorities.
The facility review shall also include emission and discharges inventories and reports to assess compliance
with standards and authorizations of:
• Sources and nature of emissions or discharges
• Process controls
• Monitoring requirements
• Compliance records
5.3.8 Site Environmental Studies
The review should include the assessment of site environmental studies to indicate the potential impact of
pollution incidents arising at the site. This will be identified by assessing the location with respect to
potentially sensitive receptors, sites of ecological importance and migration pathways for any potential
pollution sources.
5.3.9 Plans Review
The review shall include an assessment of any regulatory required plan including, but not limited to:
• Waste management
• Waste analysis
• Self auditing
• Spill and contingency plans

5.3.10 Training Programs
The review shall include an assessment of the Waste Management Contractor training programs, plans and
logs. The assessment shall determine if the Waste Management Contractor have programs in place to meet the
training requirements established in applicable regulations, permits, licenses, authorizations or registrations;
and if these programs are being met.
5.4 Waste Management Contractor Assessment Requirements
Each Waste Management Contractor shall be assessed to determine whether they are capable of providing
waste handling services sufficient to meet the requirements to mitigate the risks of the facility. The level of
assessment will be based on the amount and type of waste managed by the contractor.
5.4.1 Contractors Managing Low Volume or Low Risk Waste
For contractors that manage bio-hazardous waste, or an aggregate of less than 100 kg per month of Covidien
generated hazardous, toxic or “special focus” waste, and less than one kilogram (1 kg) per month of acutely
hazardous waste, as defined by applicable regulations, the assessment shall at a minimum include a review of
documentation from the selected Waste Managem

5.3.10 โปรแกรมการฝึกอบรมความคิดเห็นที่จะรวมการประเมินการจัดการกากท่อประปา โปรแกรมการฝึกอบรม และแผนการ
