1. Written referral form/Physician Order - If a physician order is required for your
discipline, practice act, or for reimbursement, be sure that it is obtained prior to
the evaluation
2. Payment Authorization - Know the laws and guidelines for payment in your
geographical area - Also know the payment policies of your different referral
sources
3. Referral Questions - Obtain referral questions through a verbal request(phoning
the referral source) or have them printed on the referral sheet/referral letter
4. Medical Records — Request medical records from the referral source - Review
medical records to obtain information on diagnosis, prognosis (if available), and
medical restrictions/precautions (if applicable) - Referral source may have only
an incomplete record if the referral is from someone who has only seen the
client once or twice
Have a policy on receipt of the medical records:
When do they need to be received by?
If they are not received, what is the response?
Reschedule or cancel?
5. Type of Evaluation Being Requested - Medical-Legal, job specific, baseline
6. Language/interpretation Issues - Arrange for professional interpreter as needed
