Hypertension is a chronic and complicated physical condition. The chronic condition induces a high risk factor for stroke, myocardial infarction, heart failure, chronic kidney disease, and many shorten life expectancy. The purpose of the study was to investigate factors that affected self-care of uncontrolled hypertension patients to be a basis for behavior modification to strengthen health promotion for hypertensive circulations. The population samples of 302 patients were selected from patients registered lists at the special clinic of Dokkhamtai Hospital, Phayao Province. This study modified an instrument that belonged to a questionnaire concerning to a study of the health behavior surveillance and a questionnaire for self-efficacy of health care and health behaviors to collect information from the sampled 302 hypertension patients. After trying out the instrument and testing for its reliability coefficient, the value indicated 0.92 to be an acceptable reliability coefficient. The statistical analysis concerned to the study factors of the patients characteristics, life style, knowledge of hypertension, self-efficacy, and self-care behaviors by using chi-square test and Pearson correlation coefficient test. The association analyses on personal factors and self-care behavior revealed that gender, educational level, comorbidity, alcoholic drinking history, having caregivers were associated significantly (p<0.05) with the self-care behavior. Additionally, the positive relation between the knowledge of disease and treatment and self-care behaviors were in a moderate level. The self-efficacy, social support and self-care behavior were in a high level and had shown statistically significant relationship (p< 0.01).
