Studies With Schizophrenia and Depression
The third generation of BFT studies, including studies of the effects of
BFT on schizophrenia and depression, are now well under way. These
studies are of two types. The first type attempts to employ the BFT methods
with different clinical and social populations. The second type attempts
to analyze the relative contributions of the major components of the approach.
Studies are in progress in Munich, Los Angeles (three studies, one
with Hispanic families), Providence, Naples, Athens, Manchester, Birmingham,
Southampton, Sydney, and Bonn. There is also a multicenter
collaborative study now in progress in five U.S. cities. Publication of the results
of these studies is awaited. Preliminary results suggest that most will
replicate the major findings of the University of Southern California (USC)
project. One major study completed in Salford, England, found similar
benefits for a behavioral problem-solving approach that was applied with
families (Tarrier et al. 1988b). Unfortunately, no specific benefits for social
functioning appeared to be associated with this rather less intensive approach
in the more chronically disabled population studied. Additional
field trials showed that similar results can be obtained when the approach
is applied within everyday clinical practice (Brooker et al. 1992; Curran
1988; Whitfield et al. 1988).
The large number of controlled and uncontrolled studies of BFT with
schizophrenic disorders is contrasted with the few attempts to evaluate its
effectiveness with affective disorders. Apart from the Seattle study with depressive
disorders (Jacobson 1988), the only other study in progress is comparing
the effects of adding BFT to lithium in the long-term management
of bipolar disorders (Miklowitz and Goldstein 1990).
The NIMH Treatment Strategies in Schizophrenia Collaborative Study
is attempting to examine the interaction between neuroleptic drugs and family
interventions in the aftercare of schizophrenia. Five centers—Payne Whitney
Clinic; Hillside Hospital (Long Island); Emery/Grady University; San
Francisco General Hospital; and Medical College of Pennsylvania (EPPI)—
have been selected for the study. Therapists at each center have been trained
to stabilize patients who have experienced a recent episode of schizophrenia
with optimal doses of neuroleptic drugs before randomly allocating them to
double-blind fluphenazine decanoate in one of three doses:
