5.3. Claim for Service Credit
In order to receive the service credit from DTN, the Company must:
5.3.1. send the request for service credit to DTN in writing within thirty (30) days from which the event has occurred. DTN will review all claims within thirty (30) calendar days of receipt. The Company will be informed whether the appropriate claim will be granted or rejected. If rejected, the notification will specify the basis for rejection; and
5.3.2. provide relevant documents and information, including:
(a) trouble ticket number;
(b) date and time of Trouble Ticket was opened and the service restored;
(c) kind of rebate claim;
(d) the Company contact details; and
(e) number of impacted Telecommunication Network Service port.
