Collaboration with community agencies would be important
to success in plan implementation. For example, making
use of existing transportation systems may assist patients in
attending health screening, clinic appointments, or dialysis
treatments. Local gyms and schools may offer reduced or
free educational sessions related to wellness activities such as
exercise programs or basic health education. Work places and
schools may also promote healthy lifestyles by encouraging
physical activities such as biking to school or work or taking
time to walk at lunch, making healthy food choices, and
the importance of hygiene to health (Victorian Government,
2009). By making use of existing resources within a community,
the education burden would be shared and the benefits
to the CKD patients and the members of the community
would be many.
