Current perspectives
Surgery is the oldest modality of cancer management
and, until recently, was the only treatment that
could result in cure. The potential of surgery
has been extended by modern technological
innovations, such as microsurgical techniques,
automatic stapling devices and equipment that
makes a variety of endoscopic surgical techniques
easier (Rosenberg 2008). Advances have also been
made in peri-operative management aimed at
reducing post-operative morbidity and mortality
(Widder and Pötter 2006).
Surgery not only offers a high chance of cure
for many cancers, it also plays an important role
in prevention, diagnosis, staging, palliation and
rehabilitation. In the arena of definitive surgical
treatment for primary cancer, the surgeon faces a
number of challenges. For example, surgeons should
ensure identification of patients who can be cured by
surgical resection and should also consider the effect
of the surgery on quality of life. After definitive
therapy, reconstructive techniques are sometimes necessary to aid the rehabilitation process; these
techniques often have a significant effect on function
and cosmesis
