MRI of The Brain:
History: Diffuse Large B cell lymphoma with CNS involvement S/P chemotherapy presented with weakness and numbess at both legs for 2 weeks.
Comparison: MRI brain on 8-October-2015
Findings: The study reveals new peripheral enhancing hypointense T1/ hyperintense T2 lesion at the right paramedian superior cerebellum, with adjacent mild leptomeningeal enhancement. Likely to be lymphoma involvement. DDx: subacute infarction.
No significant change of hyperintense T2 area at the right high paretal lobe and a hyper SI T2 Focus at right precentral gyrus is noted, which show some hypointense SWI area, representing encephalomalacic area with hemosiderin deposit from post-treatment change.
The remaining brain parenchyma appears normal signal intensity. NO acute infarction, acute hemorrhage, hydrocephalus, extraaxial collection of brain herniation.
Mild mucosal Thickening at bilateral ethmoid sinuses is seen.
The orbit, nasopharynx, mastoid air cell and rest paranasal sinuses are unremarkable.
Impression:
1.New peripheral enhancing lesion at the right paramedian superior cerebellum, with adjacent mild leptomeningeal enhancement, likely to be lymphoma involvement DDx: subacute infarction
2.No significant change of encephalomalacic area with hemosiderin deposit from post-treatment change at the right parietal lobe and small area of the right precentral gyrus.
