alternative ways to respond such as printing and mailing or faxing the completed survey.
This principle was followed. Respondents were given a fax number, phone number, and
address where they could reach me to either ask a question or return a survey. Several
respondents printed out the completed surveys and mailed them to me in order to
preserve their anonymity.
