What is the 6 Month Appraisal?
Guidelines begin with the orientation of an employee and continue through the development and feedback processes that occur for each employee on a 6 month basis. The appraisal performance management process is an ongoing employee development activity. The review is merely a management tool used to formally document overall performance. Questions regarding this process should be directed to Human Resources.
6 Month Appraisal Process
Leader should provide regular feedback to the new employee and complete a formal review of the employee after 6 months of employment and again at 6 months.
1. Employee completes Section I. Self-Appraisal prior to appraisal and review meeting.
2. Employee completes Section II. Career Path, all sections EXCEPT Specific Actions.
3. Leader completes Section II. Career Path, Specific Actions.
4. Leader completes Section III. Employee Rating.
5. Leader completes Section IV. Leadership Rating, only for employees in a leadership role by their Direct Manager.
6. Leader completes Section V. Overall Employee Performance Score, net score should be rounded up to a rating out of 5.
7. Leader and Employee completes Section VI. Additional Comments, together during appraisal meeting.
8. Leader and Employee sign and date the Acknowledgement.
9. Originals are sent to local HR.
