The study found that the cause of uneffective service system is the problem of Process, Man, Machine, and location. The results show the Process Re-design can reduce register time at nurse counter from 4.59 min to 3.23 min (P = 0.03) , reduce the waiting time for clinical registration from 4.05 min to 1.11 min (P = 0.01) , reduce waiting time for clinical service from 9.39 min to 4.67 min (P = 0.00) and reduce the waiting time for clinical appointment from 2.05 min to 1.33 min (P = 0.00). Moreover, the new counter’s layouts reduce the walking distance since 11 meters to 186 meters.
